We are seeking a skilled and dedicated Qualified Social Worker (QSW) to join our Adults with Learning Disabilities Team in South Tyneside. This is an exciting opportunity to work with adults with learning disabilities, supporting them to achieve positive outcomes, promote independence, and improve their quality of life.
Key Responsibilities:
Undertake assessments, care planning, and reviews in line with relevant legislation and best practice.
Work collaboratively with service users, families, and multi-disciplinary teams to deliver person-centered support.
Safeguard vulnerable adults by identifying and managing risks.
Maintain accurate and timely case records.
Ensure compliance with statutory duties, including the Care Act 2014 and Mental Capacity Act 2005.
Contribute to the team's professional development and service improvement.
What We're Looking For:
A qualified social worker, registered with Social Work England.
Experience working in adult services, ideally with individuals with learning disabilities.
Strong knowledge of relevant legislation, policies, and best practices.
Excellent communication, assessment, and organizational skills.
Ability to work both independently and collaboratively within a team.
