Expressions of Interest - Service Care Bank Team: Primary Care Pharmacist Prescribers Location: Blackburn, Preston, Bolton, Blackpool, Manchester Duties: Conduct medication reviews, ensuring safe and effective prescribing within a primary care setting. Work closely with GPs, nurses, and other healthcare professionals to optimise patient care. Manage and monitor medication therapy, providing advice and support to patients as required. Provide expert advice on the safe use of medications, including potential side effects and interactions. Review patient records and collaborate with other healthcare providers to ensure treatment plans are effective. Deliver high-quality clinical services in line with local protocols and regulatory standards. Participate in multidisciplinary team meetings and provide input on medication-related issues. Assist in the training and development of other healthcare professionals. Skills & Experience Required: Qualified Pharmacist with current GPhC registration. Experience with EMIS and S1. Independent prescribing qualification is essential. Previous experience working in a primary care or clinical setting is highly desirable. Strong knowledge of medication management, clinical guidelines, and treatment protocols. Ability to work effectively in a team and independently. Excellent communication skills, both written and verbal, with the ability to engage with patients and healthcare professionals. Strong problem-solving skills with attention to detail.
