Summary: The Senior Community Nurse will focus on Continuing Healthcare assessments, coordinating with the CHC coordinator to determine patient eligibility for NHS Funded Health Care. This role involves providing skilled nursing care, promoting health, and working collaboratively with various healthcare services to support the local population. The nurse will operate autonomously within the Community Nursing Team, delivering care in patients' homes and residential settings while adhering to public health guidelines. The position emphasizes the importance of emotional support for patients and families during challenging times.
Key Responsibilities:
- Conduct comprehensive assessments of patients’ needs to determine eligibility for NHS Continuing Healthcare funding.
- Initiate palliative care packages in collaboration with multi-agency providers.
- Provide nursing services in various community settings, including patients’ homes and residential care facilities.
- Support patients and families emotionally during terminal illness situations.
- Deliver complex information to patients and their families regarding care options.
- Lead and contribute to the development and review of care guidelines and policies.
- Participate in audit programs and apply findings to improve services.
- Assess and ensure the appropriate use of service aids and equipment.
- Maintain accurate nursing records in compliance with NMC guidelines.
- Develop and allocate teaching programs for nursing staff.
