Summary: Stockport Council is seeking an enthusiastic Occupational Therapist to join a dynamic team, offering a full-time contract with the flexibility to work from home. The role involves assessing service users with complex needs, reviewing care packages, and collaborating with partner agencies. Candidates should have experience in adult social care and be prepared for home visits as part of their responsibilities.

Key Responsibilities:

  • Assess service users and carers with complex needs related to home environments.
  • Review Packages of Care and liaise with neighbourhood teams.
  • Assess and treat patients requiring ward-based rehabilitation and discharge planning.
  • Collaborate with the Team Manager and partner agencies.
  • Provide expert advice and assessments for equipment and home modifications.
  • Recommend adaptations and ensure safeguarding of vulnerable individuals.

Key Skills:

  • Minimum of 2 years' experience as an Occupational Therapist in the community.
  • Experience in adult social care, particularly with adaptations and single-handed care.
  • Manual handling experience and knowledge of moving and handling.
  • HCPC Registration and Enhanced DBS/CRB eligibility.
  • Occupational Therapist Qualification and eligibility to work in the UK.
