This is a unique opportunity to make a significant difference by supporting service users with complex needs, promoting their independence, and ensuring the highest standards of risk management and safeguarding.
Key Responsibilities:
Conduct comprehensive needs assessments and complex care planning in line with the Care Act.
Undertake risk assessments and develop high-quality risk management plans in collaboration with service users and stakeholders.
Provide Social Supervision and prepare Ministry of Justice reports for conditionally discharged patients.
Work collaboratively with service users, families, carers, and multi-disciplinary professionals across health and social care.
Deliver Social Work interventions in inpatient secure and acute settings, as well as in the community.
Participate in safeguarding processes, including Multi-Agency Public Protection Arrangements (MAPPA).
Write detailed and professional reports for Managers' Hearings and Mental Health Review Tribunals (MHRT).
Coordinate innovative and personalized packages of care, promoting independence and improving opportunities in the community.
What We're Looking For:
A qualified Social Worker registered with Social Work England.
Experience in forensic mental health services, working with individuals with mental illness, learning disabilities, and offending histories.
Strong knowledge of:
The Care Act 2014
Risk assessment and management principles
Safeguarding protocols and MAPPA
Health & Social Care and Criminal Justice legislation
Ability to work effectively in multi-disciplinary settings while maintaining a strong Social Work perspective.
Excellent report writing and communication skills, particularly for legal and professional settings.
A commitment to empowering service users and promoting independence through person-centered planning.
