Summary: This fixed-term position as a Dietetic Family Support Worker involves supporting a pilot programme aimed at enhancing the health and well-being of children and families in Anglesey. The role requires collaboration with family support workers and the Senior Project Coordinator to deliver nutrition-based education and community interventions. The successful candidate will work autonomously while managing a caseload and engaging with various stakeholders to promote positive health changes. The position is open to both English and Welsh speakers, with a preference for those who can speak Welsh.

Key Responsibilities:

  • Plan, coordinate and deliver nutrition-based 1 to 1 and group education sessions, practical cooking skills, and community-based nutrition interventions for families.
  • Motivate, guide, and encourage families to participate in health-promoting activities, monitor their progress, and report outcomes to the team and partners.
  • Work autonomously on specific project areas, seeking advice and support as necessary.
  • Develop and maintain positive partnerships with stakeholders to promote the project and health and wellbeing principles.
  • Engage in partnership discussions and forums to share experiences and reflect on successes and areas for development.
  • Produce resources and training materials, such as presentations, information sheets, and evaluation tools.

Key Skills:

  • Good general education with sound English language skills and mathematics.
  • Relevant Level 3 qualification in Healthcare or equivalent experience, with a willingness to work towards a Level 4 qualification within 3 years.
  • Previous experience in NHS, social care, or community settings.
  • Experience using IT software and applications, such as Microsoft Office, TEAMs, and Outlook.
  • Experience working with children and families or relevant community groups.
  • Excellent communication skills, both verbal and written.
  • Ability to work independently or as part of a team.
  • Desirable: Ability to speak Welsh.
