Community Mental Health Nurse

Summary: The role of Community Mental Health Nurse involves managing a caseload of service users with complex health and social care needs, focusing on a recovery-oriented approach. The position requires previous care coordination experience and entails responsibilities such as assessment, planning, and implementation of care, as well as ensuring compliance with relevant legislation. The successful candidate will work collaboratively with a multi-disciplinary team and promote service user involvement. This is a temporary position covering maternity leave for six months.

Key Responsibilities:

• Manage a caseload of service users with complex health and social care needs.
• Conduct assessments, planning, implementation, and evaluation of evidence-based care.
• Coordinate care and assessments under the Care Programme Approach.
• Ensure compliance with Safeguarding Vulnerable Adults and Children Legislation.
• Adhere to the Mental Health Act 1983 requirements.
• Establish therapeutic relationships and implement evidence-based therapeutic interventions.
• Deliver carer assessments, education, and support in accordance with NSF requirements.
• Provide support and coverage for colleagues/team members.
