Summary: This role as a Social Worker in the Adults Locality Team involves conducting comprehensive assessments and developing person-centered care plans for adults. The position requires collaboration with healthcare providers and family members, as well as advocacy for the rights and well-being of adults in need. Key Responsibilities:

  • Conduct comprehensive assessments of adults' needs and develop person-centered care plans.
  • Provide direct social work services, including counselling, case management, and advocacy.
  • Collaborate with other professionals to coordinate care.
  • Maintain accurate and up-to-date case records and documentation.
  • Participate in team meetings and contribute to the development of policies and procedures.
  • Engage in community outreach and education initiatives.
  • Advocate for the rights and well-being of adults in need.
  • Monitor and evaluate the effectiveness of interventions and adjust care plans as necessary.

Key Skills:

  • Relevant social work qualification (e.g., MSW, LCSW).
  • Minimum of 2 years of experience working with adults in a social work capacity.
  • Strong understanding of social work theories and practices.
  • Proficient in conducting needs assessments and developing care plans.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Highly organized and detail-oriented.
  • Able to work independently and as part of a team.
  • Valid driver's license and access to a reliable vehicle (if applicable).
  • Experience working within a locality team is advantageous.
