Summary: This role as a Social Worker in the Adults Locality Team involves conducting comprehensive assessments and developing person-centered care plans for adults. The position requires collaboration with healthcare providers and family members, as well as advocacy for the rights and well-being of adults in need. The role also includes maintaining accurate case records and participating in community outreach initiatives.
Key Responsibilities:
- Conduct comprehensive assessments of adults' needs and develop person-centered care plans.
- Provide direct social work services, including counselling, case management, and advocacy.
- Collaborate with other professionals to coordinate care.
- Maintain accurate and up-to-date case records and documentation.
- Participate in team meetings and contribute to the development of policies and procedures.
- Engage in community outreach and education initiatives.
- Advocate for the rights and well-being of adults in need.
- Monitor and evaluate the effectiveness of interventions and adjust care plans as necessary.
Key Skills:
- Relevant social work qualification (e.g., MSW, LCSW).
- Minimum of 2 years of experience working with adults in a social work capacity.
- Strong understanding of social work theories and practices.
- Proficient in conducting needs assessments and developing care plans.
- Excellent communication, interpersonal, and problem-solving skills.
- Highly organized and detail-oriented.
- Able to work independently and as part of a team.
- Valid driver's license and access to a reliable vehicle (if applicable).
- Experience working within a locality team is advantageous.
