Summary: The Young Person’s Resilience Worker at Change Grow Live is responsible for delivering interventions that empower young people to reduce risk and build resilience within a multi-disciplinary team. This role focuses on supporting young individuals through their treatment and recovery journeys, promoting holistic health and well-being. The position requires collaboration with community partners and internal teams to address the dynamic needs of young people. The role is based in Sheffield and is a full-time, 24-month fixed-term contract.

Key Responsibilities:

  • Support young people from entry into the service through their treatment and recovery journey.
  • Provide screening, assessment, psychosocial interventions, integrated care planning, and post-treatment support.
  • Identify and respond to potential safeguarding issues.
  • Reduce drug and alcohol-related harm to young people and the community.
  • Promote carer, service user, and community involvement.
  • Advocate for access to partnership services.
  • Work with young people to achieve positive health and well-being outcomes.
  • Work flexibly across various sites as required.
  • Provide targeted and specialist 1-2-1 work.
  • Consult and support exploitation services in the city.
  • Attend relevant meetings to offer consultation and support to professionals.

Key Skills:

  • Experience working with young people and knowledge of their issues.
  • Understanding of safeguarding concerns and the Fraser Competence framework.
  • Excellent understanding of drug and alcohol issues and related experience.
  • Knowledge of evidence-based practices in young people’s substance misuse treatment.
  • Strong verbal and written communication skills with high IT competency.
  • Able to maintain accurate records and meet deadlines for information submission.
  • Excellent team working and interpersonal skills.

As a Young Person’s Resilience Worker- Child Exploitation & Vulnerable People Lead , you’ll be part of a multi-disciplinary team delivering interventions that empower young people to reduce risk, build resilience, and lead safer, healthier lives. You will be working within an all age, whole family substance use service across the county. Our approach is centered around a resilience and strengths-based model of support that incorporates some core principles around preventionand earlier intervention, harm reduction and person-centered support. You will engage young people in a range of interventions, utilising strengths based and young person-centered approaches, empowering them to reduce risk, increase resilience and lead a safe, happy, and healthy life. You will work in an integrated way and collaborate with our with our key community partners and internal teams in providing the best response to the emerging and dynamic needs of young people.

Where Sheffield Contract Type 24 Month Fixed Term Contract Full Time Hours 37.5 per week Full Time Salary Range £27,861.26 - £32,002.35* Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.

About The Role Supporting young people from point of entry into the service and through their treatment/recovery journey Providing screening, assessment, psychosocial interventions, integrated care planning, post treatment support, including transfer of care Identifying and responding effectively to potential safeguarding issues Reducing drug and alcohol related harm to young people and the wider community Promoting carer, service user and community involvement Providing advocacy for access to partnership services Working with young people to support positive, holistic outcomes in relation to their health and wellbeing, enabling them to lead safe, healthy, and purposeful lives reducing risk and increasing resilience Working flexibly across sites where required Targeted and Specialist 1-2-1 work Provide consultation and support to Amber Project, CSE and other exploitation services in the city. Build, strengthen and maintain links with exploitation services in the city to improve pathways and increase referrals Attend MACE and other relevant meetings to offer consultation and support to professionals.

About you Experience of working with young people and knowledge of the issues they face Knowledge of safeguarding concerns in relation to children and young people and the Fraser Competence framework Have an excellent understanding of drug and alcohol issues and experience of working within a related field Knowledge of working with evidence-based practice around young people’s substance misuse treatment services and methods, including relevant best practice guidance An accomplished communicator, both verbal and written with a high degree of personal IT competency Be able to accurately update and maintain records and to work to deadlines for the submission of information, e.g., reports Have excellent team working and interpersonal skills, maintaining a highly cooperative approach to supporting colleagues in delivering service objectives.
