Eden Brown Synergy is currently recruiting for a Qualified Social Worker to join the Hospital Social Work Team in Wolverhampton. This part-time role is perfect for experienced social workers looking for flexibility and variety in their work.

Role Responsibilities:
Support hospital discharges by completing assessments under the Discharge to Assess (D2A) model, covering pathways 1, 2, and 3.
Collaborate with health professionals and care providers to develop effective care plans.
Conduct visits to care homes and service users' homes as required.
Work in a hybrid capacity, combining home working, on-site visits, and office-based work.
Ensure timely, person-centered assessments and interventions that meet statutory requirements.
Candidate Requirements:
Qualifications: Degree in Social Work, Dip SW, CQSW, or equivalent.
Experience: Previous experience in hospital discharge planning and adult social care.
Registration: Must be registered with Social Work England (SWE).
Driving License: A valid driving license and access to a vehicle are essential for this role.
Skills: Strong organizational, assessment, and communication skills.
