Summary: The Transition Manager will lead a critical role in a major NHS transformation programme, overseeing the transition strategy from bid and mobilisation to operational handover. This position requires extensive experience in managing service transitions and operational readiness within healthcare settings. The role involves collaboration with various stakeholders to ensure successful service adoption and integration into live environments. The contract is for 8–12 months and is classified as outside IR35.

Key Responsibilities:

  • Support bid and procurement activities by shaping transition, service introduction, and operational readiness strategies.
  • Develop and implement programme-to-BAU transition plans, governance frameworks, and operating models.
  • Establish and oversee operational readiness, service acceptance, and go-live assurance processes.
  • Lead service introduction activities across multiple workstreams and delivery partners.
  • Define and govern hypercare, early-life support, and service stabilisation arrangements.
  • Coordinate onboarding activities across customer services, service desks, operational teams, and support functions.
  • Manage supplier onboarding, support integration, and third-line support readiness.
  • Drive knowledge transfer, training, and operational handover activities.
  • Conduct service acceptance reviews and readiness assessments to support successful go-live decisions.
  • Manage transition risks, dependencies, issues, and governance throughout the programme lifecycle.

Key Skills:

  • Extensive Transition Management experience within large-scale transformation or business change programmes.
  • Proven track record of transitioning complex services into live operational environments.
  • Strong expertise in Service Introduction, Service Transition, and Operational Readiness.
  • Solid understanding of ITIL principles and Service Management best practices.
  • Experience operating within multi-supplier delivery and support environments.

Transition Manager Major NHS Transformation Programme | 8–12 Month Contract | Bid, Mobilisation & Service Transition Outside IR35 Contract – Warwickshire Hybrid – Max 1 day a week

We are seeking a highly experienced Transition Manager to play a pivotal role within one of the UK's most significant healthcare transformation programmes. Working across the full programme lifecycle—from bid and mobilisation through to delivery and operational handover—you will be responsible for designing and leading the transition strategy that ensures successful service adoption and seamless integration into live operational environments. This is a high-profile leadership position with responsibility for operational readiness, service introduction, supplier onboarding, support model implementation, and the effective transition of programme outcomes into Business-as-Usual (BAU) operations. You will collaborate closely with programme leadership, operational teams, service management functions, customer services, and third-party suppliers to ensure services are fully prepared for launch and capable of delivering value from day one.
