Summary: Tower Hamlets Council is looking for a Qualified Social Worker to join their Hospital Social Work Team, focusing on safe and timely hospital discharges under the Discharge to Assess (D2A) Model. The role requires experience in hospital discharge processes and multi-agency collaboration. The position offers a supportive environment with flexible working arrangements. Immediate availability is preferred for this fast-paced role.
Key Responsibilities:
- Carry out assessments for hospital patients to ensure safe and timely discharge.
- Implement Discharge to Assess (D2A) and Home First policies.
- Manage a quick turnover of cases with no waiting list.
- Ensure seamless case transfer to Locality Teams post-discharge.
- Work in partnership with NHS, social care teams, therapists, and voluntary sector organisations.
- Attend multi-disciplinary team meetings to support effective discharge planning.
- Undertake Care Act 2014 assessments, Mental Capacity Assessments (MCA), and Best Interest Decisions (BID).
- Identify safeguarding concerns and complete investigations where necessary.
- Flexible working arrangements, including working on-site and from home.
- Participate in a supportive team culture with strong management structure.
Key Skills:
- Qualified Social Worker registered with Social Work England.
- Proven experience in hospital discharge, D2A, and multi-agency working.
- Knowledge of the Care Act 2014, Mental Capacity Act 2005, and safeguarding procedures.
- Strong assessment and crisis intervention skills.
- Hybrid working flexibility - able to work on-site and remotely as required.
- Immediate availability preferred.
