Key Responsibilities:
Conduct Care Act assessments for individuals transitioning from hospital care to step-down settings.
Work closely with MDT professionals to develop tailored support plans that facilitate safe discharge back to the community.
Coordinate and arrange care packages and support services as required.
Perform functional assessments, Continuing Healthcare (CHC) assessments, and mental capacity assessments when necessary.
Ensure all assessments and interventions are person-centered and compliant with relevant legislation and best practices.
Essential Requirements:
Professional qualification in Occupational Therapy and current HCPC registration.
Experience conducting Care Act assessments and working within adult social care settings.
Strong understanding of discharge planning and rehabilitation pathways.
Ability to work collaboratively with MDT professionals and external care providers.
Excellent assessment, communication, and problem-solving skills.
