Key Responsibilities:

• Undertake timely hospital discharge planning assessments to ensure safe transitions back into the community.
• Conduct Mental Capacity Assessments (MCA) and ensure compliance with the Care Act 2014 & Mental Capacity Act 2005.
• Identify and respond to safeguarding concerns, following Section 42 Enquiries.
• Work in partnership with NHS teams, occupational therapists, and other professionals to develop support plans.
• Maintain accurate case records using Liquidlogic Adults System (LAS).
• Provide professional social work intervention, support planning, and risk management for vulnerable adults.

Key Skills:

• Registered Social Worker with Social Work England.
• Previous experience in a hospital discharge team or adult social care setting.
• Strong knowledge of Care Act 2014, Mental Capacity Act, and Safeguarding Adults Framework.
• Ability to work in a fast-paced environment and manage complex cases.
