Summary: This role is for a Specialist Community Heart Failure Nurse within the Community Heart Failure Team, focusing on delivering complex care for patients with heart failure in the London Borough of Wandsworth. Key Responsibilities:

This is a great opportunity for you to join our established Community Heart Failure Team as a specialist nurse Band 7 for a years Fixed Term or secondment! The service covers the London Borough of Wandsworth and provides complex care for people with Heart failure involving supporting secondary and primary care, community integrated services and other healthcare professionals. Our specialist nurses are required to deliver complex Heart Failure management together with providing education to patients to improve self-management of their condition.
Main duties of the job: You will primarily be responsible for managing a caseload of patients with Heart Failure in the community. To support this, you will liaise with other members of the heart failure management team across primary and secondary care to deliver evidence based care to patients with Heart Failure. You will also co-ordinate an education programme for community health professionals and act as a resource for advice. You will use your specialist Heart Failure knowledge and skills to safely identify, maintain and manage a caseload of heart failure patients, providing them with appropriate treatment, support and advice.
