Description

We are currently recruiting a Social Worker - Level 2/3 for Hospital Discharges. To provide assessment, care management and social work services to users and carers arising out of the duties and powers under legislation, and Group Policy. To work following the guidelines set out by local management and to make appropriate use of group and independent resources.

Responsibility

To assess the needs of People who are in hospital who may require Community Care Services, on request, and to provide Care Plans and arrange for the provision of services where the Eligibility Criteria are met.

Qualification – Essential

  • GCSE Grade (or equivalent) in English Language and Mathematics.
  • DipSW Qualified or Equivalent
  • HCPC Registration / GSCC Registered
  • Proof of Qualifications
  • Advanced – Microsoft Office (Word, Excel, Outlook), social media

Qualifications, Knowledge, and Experience Requirements - Criteria to be Evaluated at the Shortlisting Stage.

Successful candidates will have:

  • The ability to be assertive and challenging to safeguard and promote the welfare of children.
