Summary: This role is for a Band 6 District Nurse/Senior Community Nurse within a multi-disciplinary care team, requiring a UK driving license and access to a car. The successful candidate will be responsible for providing skilled nursing care and promoting health within the community, working flexible hours as needed. Key Responsibilities:

• Manage a caseload, including assessment and development of care programs.
• Lead and supervise community staff nurses and support junior team members.
• Provide skilled nursing care to residents, promoting health and independence.
• Collaborate with General Practices, Social Services, and other services to meet community needs.
• Implement and monitor the Out of Hospital model of care.
• Work as an autonomous practitioner within the Community Nursing Team.
