Key Responsibilities:
- Work in partnership with educational staff, health care professionals, and other agencies to provide advice and support to children & young people, their parents, and carers.
- Undertake training/updates related to public health issues to meet local and national Department of Health targets.
- Conduct training sessions for other professionals on relevant health or medical issues and assist children and young people in managing their health conditions.
- Perform health assessments on children & young people and ensure referrals, treatments, and reviews are conducted as per Trust policies.
- Develop high-quality care packages for children & young people with medical needs in collaboration with health professionals and other agencies.
- Identify children & young people requiring medical follow-up planning.
Key Skills:
- Registered Nurse with Specialist Community Public Health Nursing (SCPHN) registration or working towards it.

As an integral member of the Bi Borough KCW School Health service, you will be responsible for delivering the Healthy Child Programme in collaboration with the wider school health team. We also have Specialist Leads for Emotional Health & Mental Health Well-being, a Violence Against Women and Girls (VAWG) Lead, a Relationship and Sexual Education Team Lead and RSHE specialist workers as part of our School Health service. We offer excellent professional development opportunities along with formal one to one supervision and annual appraisal.
We are advertising for a post in our Westminster Team based in Central London (Soho Square). Full and part time hours considered; year round and term time only opportunities available. Please state in your application your preferred working pattern.

- To undertake agreed health assessments on children & young people and ensure referrals, treatments and reviews are carried out as recommended and in accordance with Trust policies and procedures.
