Job Overview An exciting opportunity has arisen for a Salaried General Practitioner to join the team at Hwb Iechyd Cybi on a fixed-term basis to cover maternity leave . Hwb Iechyd Cybi is a forward-thinking, multidisciplinary primary care hub based in Holyhead, serving a diverse and vibrant community in Anglesey. Main duties of the job The successful candidate will:

• Provide high-quality, safe, and effective primary care services to the registered patient population
• Undertake routine and urgent consultations, delivered face-to-face, via telephone, and digitally as appropriate
• Manage a diverse clinical caseload, including acute presentations, chronic disease management, and preventative care
• Work collaboratively with a multidisciplinary team including Advanced Nurse Practitioners, Practice Nurses, Pharmacists, and Care Coordinators
• Contribute to continuity of care and patient experience through proactive clinical management and follow-up
• Participate in practice meetings, clinical governance activities, and quality improvement initiatives
• Support the development of innovative models of care within a managed practice setting
• Maintain accurate and contemporaneous clinical records in line with organisational and professional standards

The role may also include opportunities to:

• Contribute to teaching and supervision (where appropriate)
• Engage in service development aligned with “Care Closer to Home” principles
• Support population health management and preventative care initiative
