Key Responsibilities:

  • Provide high-quality, safe, and effective primary care services to the registered patient population
  • Undertake routine and urgent consultations, delivered face-to-face, via telephone, and digitally as appropriate
  • Manage a diverse clinical caseload, including acute presentations, chronic disease management, and preventative care
  • Work collaboratively with a multidisciplinary team including Advanced Nurse Practitioners, Practice Nurses, Pharmacists, and Care Coordinators
  • Contribute to continuity of care and patient experience through proactive clinical management and follow-up
  • Participate in practice meetings, clinical governance activities, and quality improvement initiatives
  • Support the development of innovative models of care within a managed practice setting
  • Maintain accurate and contemporaneous clinical records in line with organisational and professional standards
  • Contribute to teaching and supervision (where appropriate)
  • Engage in service development aligned with “Care Closer to Home” principles
  • Support population health management and preventative care initiatives

Key Skills:

  • Full GMC Registration with a licence to practise and entry on the GP Register
  • Certificate of Completion of Training (CCT) in General Practice / Certificate Confirming Eligibility for General Practice Registration (CEGPR), or equivalent
  • Included on a Medical Performers List or eligible for inclusion
  • Evidence of continuous professional development
  • Proven ability to handle a busy and varied primary care caseload
  • Ability to take independent clinical decisions and seek advice from senior doctors
  • Commitment to team approach and multi-disciplinary working
  • Effective counselling and communication skills
  • Understanding of clinical risk management and clinical governance

Job Overview An exciting opportunity has arisen for a Salaried General Practitioner to join the team at Hwb Iechyd Cybi on a fixed-term basis to cover maternity leave . Hwb Iechyd Cybi is a forward-thinking, multidisciplinary primary care hub based in Holyhead, serving a diverse and vibrant community in Anglesey.
