Job Title: Community Care Lead Nurse (Fixed Term Contract to cover mat leave)
Reporting to: Community Care Nurse Manager
Location: Remote based within Perth / Dundee (Travel within Scotland required)
Salary: Up to £42,500 + up to £10,000 pa Bonus + car allowance
Hours: Core hours are Monday – Friday 09:00 – 17:30 (37.5 hours per week)

Who are you? You will be a Qualified RN or RNLD Nurse in adult care and will be dedicated and compassionate about making a difference in your community. You will want to be part of a national team of nurses dedicated to providing expert care in the community allowing care and support to individuals with complex needs, enabling them to be cared for in the comfort of their own home.
What will you do? Whilst a full job description can be supplied here are a few of the key responsibilities for the role: You will play a pivotal role in community engagement, being physically visible and accessible to service users, their families, and carers. Establishing a strong presence within the community is crucial to ensuring accessibility and fostering open communication and trust. You will be responsible for devising, implementing, and assessing care plans, risk assessments, and associated documentation. Using a person-centred approach, you will develop comprehensive care plans and regularly update them to reflect the evolving needs of service users. Additionally, you will work alongside and support the Registered Manager to assess and review all referrals, ensuring seamless coordination and integration of care services. Service safety and competency are paramount at Newcross Healthcare. You will act as a trusted voice and advisor, contributing to competency sign-offs to uphold high standards of care. The role involves supporting, monitoring, and being accountable for delegated clinical tasks, overseeing Community Care Leads and Healthcare Workers, and ensuring adherence to clinical competencies. Leadership, support, supervision, and direction are key components, as is maintaining clinical training and competencies through ongoing support. Out-of-hours support and on-call responsibilities will also be part of the role.
Skills Required: Qualified Nurse with current and relevant RN or RNLD registration. Experience in Adult Nursing Complex/Clinical care would be beneficial 2 years post-qualification clinical working experience. Up-to-date understanding of national standards and regulations (CQC, Care Inspectorate Scotland/Wales). Competence in technology and remote work. Understanding of NMC Codes of practice and Continuous Professional Development. Full driving licence and willingness to travel.
