The focus of the role is to bridge the gap of health inequalities and provide education to the communities and professionals of the social determinants within the vulnerable groups.As an autonomous practitioner the post holder is responsible for the care delivered, demonstrating critical thinking, sensitivity and skills in clinical decision-making in the management of patients.This post is fixed term/secondment for 12 moths due to maternity cover. If you are interested in applying for the secondment position, you must obtain permission from your current line manager prior to applying for this post.The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.Main duties of the jobSupport the Senior Nurse in delivering and supporting effective care to patients.Assist with managing the healthcare service across the socially vulnerable groups that we support.To deputise for nursing colleagues during any periods of absence.To represent the primary and community care viewpoint at operational and strategic meetings when necessary.Work closely with the wider multidisciplinary team to ensure safe effective delivery of health care, including mental health and public health services.To act as an accountable role model and clinical expert for healthcareResponsibility for the promotion and use of the audit cycle within clinical practice.Support the development and implementation of Healthcare Policies and procedures.Responsibility of offering the initial assessment/completion of the All Wales Blue book to Asylum seekers living in Newport.Responsibility of providing cultural awareness training and needs of the socially vulnerable groups to other disciplines for example g.p practices staff, mental health teams and Accident and Emergency staff.Work in conjunction with other disciplines in the assessment, planning, implementation and evaluation of healthcare service provision to ensure a smooth patient pathway through the services.Working for our organisationAneurin Bevan University Health Board is a multi-award-winning NHS organisation with a passion for caring. Join us on our journey to pioneer new ways of working and deliver a world-class healthcare service fit for the future.Qualifications & KnowledgeEssential criteriaRegistered General Nurse with NMCEvidence of certified/diploma level training in clinical condition management (or equivalent).Understanding of the clinical governance strategy and how to implement CG principles in order to influence the quality of care.Knowledge of health inequalities faced by socially vulnerable groupsDesirable criteriaTeaching qualification.Independent and/or Supplementary nurse prescribing qualificationSpecialist Practitioner QualificationExperienceEssential criteriaEvidence of successful management of change.Experience of quality initiatives such as clinical benchmarking.Ability to support and mentor staffDesirable criteriaPrevious experience in a leadership role.Experience of managing a budget.
