Summary: The Post Diagnostic Support Link Worker role at NHS Greater Glasgow and Clyde involves providing essential support to individuals diagnosed with dementia and their families, enhancing the existing services within the Renfrewshire Older Peoples Community Mental Health Service. This fixed-term position lasts for 18 months and requires effective leadership within the Post Diagnostic Support service, working Monday to Friday from 9:00 am to 5:00 pm. The role aims to help individuals live well with dementia by offering advice and support tailored to their needs.
Key Responsibilities:
- Provide Post Diagnostic Support in accordance with the 5 Pillar Model for dementia patients.
- Work with families for a minimum of one year to offer advice and support.
- Ensure effective leadership within the Post Diagnostic Support service.
- Enhance support available from the Renfrewshire Older Peoples Community Mental Health Service.
Key Skills:
- Experience in mental health services, particularly with older adults and dementia care.
- Strong communication and interpersonal skills.
- Ability to provide leadership and support within a community health setting.
- Knowledge of the 5 Pillar Model for dementia care.
