- Be a car driver with access to a vehicle for community-based visits and service delivery
- Hold current HCPC registration
- Have experience working within a Multidisciplinary Team (MDT) environment
- Have previous rehabilitation experience, which would be highly advantageous
- Possess post-graduate clinical experience, as the service manages complex caseloads and requires confident clinical decision-making
This role involves working across all Intermediate Care pathways, including:
- Reablement services
- Rapid Response pathways
- Rehabilitation beds and community rehabilitation services
Candidates must also demonstrate previous community experience and the ability to work independently within patients’ homes and community settings while delivering high-quality, patient-centered care. The ideal applicant will be adaptable, organized, and capable of managing complex patient needs within fast-paced healthcare environments.
