Summary: The Part Time Community Connector – Mental Health role involves supporting GP surgeries in Surrey by facilitating access to mental health services and community resources. The position is integral to the development of integrated primary care mental health services, requiring collaboration with various healthcare professionals. The role emphasizes early intervention and support for patients with mental health needs. This is a part-time position with a fixed-term contract of 16 months.
Key Responsibilities:
- Support the ongoing development and mobilisation of integrated primary care mental health services.
- Act as the first point of contact for GPs seeking support for patients with mental health presentations.
- Provide early intervention and promote early triage, assessment, and formulation of care plans.
- Coordinate and support patients in accessing community services and resources.
- Work collaboratively with a multidisciplinary team including GPs, nurses, and mental health practitioners.
Key Skills:
- Community experience in drug, alcohol, and mental health work.
- A Diploma or degree in a relevant subject (Counselling, Psychology, Mental Health, Social Work, Health and Social Care).
- Experience with signposting to community support services.
- Strong written and verbal communication skills.
- Proficiency in Microsoft Word and Excel.
- Ability to travel within the area, with own transport preferred.
