Summary: The role is for a Qualified Occupational Therapist to join Blackburn and Darwen Council's Community-based Assessments and Interventions team. This full-time position involves delivering community assessments and interventions to support individuals' independence and well-being. Key Responsibilities:
- Delivering essential community-based assessments and interventions for individuals requiring equipment, minor and major adaptations to support their independence and well-being.
- Conduct community assessments in a local authority setting, identifying needs and recommending both standard and specialist equipment.
- Plan and facilitate minor and major adaptations to enhance safety and functionality within clients' homes.
- Perform manual handling assessments, ensuring safe practices and compliance with relevant guidelines.
- Collaborate with clients, families, and other professionals to create individualised support plans.
- Maintain thorough and accurate records in line with professional and legal standards.
Key Skills:
- Occupational Therapist Degree or equivalent
- Enhanced DBS
- HCPC registration
- Experience with adults
