As a Community Care Coordinator, you will support clients through challenging times, empowering them toward recovery and independence. You will manage a caseload, coordinate holistic care, and deliver recovery-focused interventions within a supportive multidisciplinary team. Please note: This is a fixed term 12 month contract to provide maternity cover. As a band 6 clinician you will be expected to act as a role model and resource for our less experienced staff. You will also have opportunity to be involved in service delivery improvements and innovations. You will be offered regular supervision and support.
