We are looking for an enthusiastic and motivated Occupational Therapist to join our dynamic team. The post holder will work as a Band 6 Occupational Therapist within the Early Intervention Service on a fixed-term secondment until 31 August 2027. This role focuses on supporting individuals living with long-term conditions to improve self-management, maintain independence, and optimise participation in daily life.

Main duties of the job

  • To provide specialist occupational therapy assessment and intervention for individuals with long-term conditions, focusing on the impact on daily functioning, roles, and quality of life.
  • To support service users to develop effective self-management strategies, including pacing, fatigue management, energy conservation, and symptom control.
  • To work collaboratively with individuals to set meaningful, person-centred goals that promote independence, wellbeing, and participation in everyday activities.
  • To deliver early intervention aimed at preventing functional decline, reducing hospital admissions, and minimising long-term reliance on health and social care services.
  • To facilitate behaviour change and promote healthy routines through coaching approaches, motivational interviewing, and education.
  • To provide advice, education, and support to carers and families to enhance the individual’s ability to self-manage their condition.
  • To work autonomously in managing a defined caseload, prioritising referrals and interventions in line with service criteria and clinical needs
  • To identify risks and escalate concerns appropriately, ensuring safe and effective care delivery.
  • To maintain accurate, comprehensive, and timely clinical records in line with professional, legal, and organisational standards.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
