We have an exciting opportunity for a dedicated professional to join our Reablement / Hospital Social Work Team within the Integrated Discharge Hub. Based in Livingston in Scotland 36 hours per week
Role Overview
This role is based within a busy hospital setting, focusing primarily on reablement and assessment work . You will play a key role in supporting patients to regain independence and ensuring safe, effective discharge pathways back into the community. Working as part of the Integrated Discharge Hub, you’ll collaborate closely with multidisciplinary teams and community services to deliver high-quality, person-centred care.
We’re seeking someone with:
