Summary: The Occupational Therapy Care Manager will join the Community Independence Service (CIS) Reablement Team on a locum basis, focusing on delivering skilled occupational therapy interventions to promote client independence. This dual role involves clinical responsibilities and managing care assessments for clients requiring ongoing support post-reablement. The position offers flexible working hours from Monday to Friday, 9 AM to 5 PM.
Key Responsibilities:
- Deliver skilled Occupational Therapy interventions to residents in their homes, supporting independence and recovery.
- Manage a caseload for up to six weeks, creating and implementing goal-focused reablement plans.
- Conduct holistic assessments to evaluate clinical and personal needs, ensuring comprehensive support.
- Develop and execute intervention plans, including complex movement and handling assessments.
- Recommend, order, and provide adaptive equipment to enhance independent living.
- Collaborate with a multidisciplinary team to ensure coordinated and effective service delivery.
- Maintain accurate and up-to-date case records using the Mosaic system.
Key Skills:
- HCPC-registered Occupational Therapist.
- Experience in adult social care or community settings.
- Experience in complex movement and handling assessments and care planning (training and supervision available).
