Nurse Associate - Band 4 - Harrow ICS Community Nursing, Permanent: 37.5 HRS per Week

Are you a Nurse Associate nurse passionate about community nursing? Are looking for a new challenge? If youve answered yes , wed love to talk to you about joining one of our teams. We currently have A Band 4 Nurse Associate vacancy in Harrow.

In Central London Community Healthcare NHS Trust (CLCH), community healthcare is our focus and our passion. We champion the role of community health professionals to make sure our patients get great care closer to home and we have recently been rated as good by the Care Quality Commission. We believe community nursing gives dedicated healthcare professionals the opportunity to spend more time with the patients in their own home environment and work collaboratively with other clinicians to provide more coordinated care.

You will be working independently under the guidance and direction of a District Nurse Team leader to provide cost effective, evidence based and safe care to our patients in their own homes. Applicants must be registered with the NMC as a nurse associate and have excellent communication, clinical skill and team working skills. Experience of working in the community is desirable but not essential.

The role of the Nursing Associate is to work with the multi professional team and the Registered Nurse to assess, plan, organise and deliver clinical care in line with service and patient needs, as agreed with supervising clinicians. The nursing associate has a breadth of knowledge across the lifespan and across the fields of nursing, providing holistic and person-centred care and support for people of all ages and in a variety of settings. The nursing associate works independently under the leadership of registered nurses , working within the sphere of nursing and care and within all aspects of the nursing process. Nursing associates are equipped with the knowledge, understanding, skills, attitudes, and behaviours relevant to employment as a nursing associate and will work to a nationally recognised Nurse and Midwifery (NMC) code of conduct and must meet the NMC standards of proficiency.

Assessment and planning of delegated care

Monitoring of long-term conditions and well-being of service users including escalation of interventions when necessary

Assist with the implementation and monitoring of clinical care and monitor outcomes

Administering of Medication in accordance with regulatory guidelines for Nursing Associates
