Preparing For Adulthood, Autism And Learning DisabilitiesA Local Authorities newly developed Preparing for Adulthood (PFA) and Learning Disability/Autism (LDA) team is offering an exciting opportunity for experienced Social Workers to join our dynamic and supportive team. Established in September 2023, the team is dedicated to delivering comprehensive and timely assessments for young people transitioning into Adult Services, as well as adults with Learning Disabilities, Autism, and neurodiverse conditions.As a Social Worker within the PFA/LDA team, you will be responsible for managing a caseload of approximately 20 cases, depending on complexity. You will engage in robust case management, conducting Care Act 2014 assessments, mental capacity evaluations under the Mental Capacity Act 2005, and safeguarding interventions in line with the Care Act. Your role will also involve attendance at Education, Health, and Care Plan (EHCP) reviews, ensuring that our young people receive the support they need during key transitions.Key ResponsibilitiesCare Act 2014 Assessments: Conduct assessments to determine eligibility for adult social care services, focusing on individuals with Learning Disabilities, Autism, and neurodiversity.Mental Capacity Assessments: Apply the Mental Capacity Act 2005 principles to assess decision-making abilities and ensure the rights and preferences of individuals are respected.Safeguarding: Lead safeguarding enquiries in accordance with the Care Act 2014 to protect vulnerable adults from abuse and neglect.Case Management: Develop, implement, and review personalised support plans, coordinating services to meet the needs of individuals with high-level support requirements.Duty Rota Participation: Contribute to the team s duty system, ensuring prompt response to urgent needs and providing support to colleagues.EHCP Reviews: Attend and contribute to Education, Health, and Care Plan (EHCP) reviews for young people transitioning from children s services to adult services.About You:The ideal candidate will have a minimum of 2 years post-qualifying experience (PQE) in a statutory Adult Social Work role, with specific experience in Learning Disabilities, Autism, and/or Transitions teams. You should have a strong understanding of UK legislation and statutory responsibilities, particularly the Care Act 2014 and the Mental Capacity Act 2005.
