Neuro Community Physiotherapist- Hertforshire

Summary: This role is for a Band 6 Physiotherapist specializing in neuro and stroke rehabilitation within a community setting in Hertfordshire. The position offers flexibility, strong multidisciplinary team support, and the opportunity to significantly influence patient recovery. Candidates will be responsible for conducting assessments and developing rehabilitation plans while working closely with other healthcare professionals. The role is ongoing with full-time hours, though part-time applicants are also considered.

Key Responsibilities:

• Conduct detailed neuro/stroke physiotherapy assessments.
• Develop and deliver patient-centered rehabilitation plans.
• Liaise with the multidisciplinary team including occupational therapists, nurses, and speech & language therapists.
• Advise patients, families, and carers on exercises and ongoing management.
• Maintain accurate and up-to-date documentation.

Key Skills:

• HCPC registration as a Physiotherapist.
• Experience working with stroke/neuro patients, ideally in the community.
• Strong clinical reasoning, rehabilitation planning, and communication skills.
• Ability to work autonomously and as part of a multidisciplinary team.
• Car driver with access to a vehicle (preferred for community travel).
