We are seeking a dedicated and experienced Qualified Social Worker to join our Older Persons Mental Health Team in Middlesbrough. In this role, you will provide vital support to adults over 65 with mental health needs, primarily focusing on those with dementia. Your role will be critical in ensuring that older adults receive the care and support they need to maintain their well-being and quality of life.Key ResponsibilitiesCase Management & Reviews: Manage and cover case work and reviews for older adults with mental health needs.
