Summary: The role of Locum Band 6 Community Mental Health Practitioner involves managing a caseload of adults with complex mental health needs within an Adult Community Mental Health Team. The position requires providing assessment, care coordination, and therapeutic interventions in a multidisciplinary setting. Candidates must be registered as a Mental Health Nurse, Social Worker, or Occupational Therapist with relevant community mental health experience. The contract is locum, full-time, and offers a competitive hourly rate.
Key Responsibilities:
- Manage a defined caseload, providing recovery-focused care coordination under the CPA framework
- Undertake comprehensive mental health and risk assessments
- Develop and review individualised care plans in collaboration with service users and carers
- Deliver evidence-based interventions and monitor treatment effectiveness
- Liaise with GPs, acute services, social care, and voluntary organisations
- Participate in regular MDT meetings, supervision, and service development
- Maintain accurate and timely clinical records in line with trust policies
Key Skills:
- NMC registration as a Mental Health Nurse (RMN) or Social Work England registration as a Social Worker or HCPC registration as an Occupational Therapist
- Minimum of 12 months’ post-qualification experience in community mental health services
- Knowledge of relevant mental health legislation (e.g. MHA, MCA, safeguarding)
- Excellent clinical assessment, communication, and organisational skills
- Full UK driving licence and access to a vehicle (community visits required)
- Enhanced DBS check (mandatory)
