Summary: The role of Locum Band 6 Community Mental Health Practitioner involves managing a caseload of adults with complex mental health needs within a multidisciplinary team. The position is open to Registered Mental Health Nurses and Qualified Occupational Therapists, requiring a focus on recovery-oriented care and comprehensive assessments. The practitioner will engage in care coordination, therapeutic interventions, and collaboration with various health and social care professionals. This is a full-time locum position based in Preston, Lancashire, with a contract length of 3 to 6 months.

Key Responsibilities:

  • Manage a defined caseload, providing recovery-focused care coordination under the CPA framework
  • Undertake comprehensive mental health and risk assessments
  • Develop and review individualised care plans in collaboration with service users and carers
  • Deliver evidence-based interventions and monitor treatment effectiveness
  • Liaise with GPs, acute services, social care, and voluntary organisations
  • Participate in regular MDT meetings, supervision, and service development
  • Maintain accurate and timely clinical records in line with trust policies

Key Skills:

  • NMC registration as a Mental Health Nurse (RMN) or HCPC registration as an Occupational Therapist
  • Minimum of 12 months' post-qualification experience in community mental health services
  • Knowledge of relevant mental health legislation (e.g. MHA, MCA, safeguarding)
  • Excellent clinical assessment, communication, and organisational skills
  • Full UK driving licence and access to a vehicle (community visits required)
  • Enhanced DBS check (mandatory)
  • Experience using electronic patient records (e.g. RiO or SystmOne) - desirable
  • Training in psychosocial interventions or CBT approaches - desirable
  • Familiarity with trauma-informed or recovery-oriented care models - desirable
