Key Responsibilities:
- Independently assessing, diagnosing, planning, implementing, and evaluating treatment and interventions for patients with undifferentiated and undiagnosed conditions.
- Providing expert clinical care for patients presenting with acute illnesses, long-term conditions, and complex health needs.
- Working autonomously to see patients in clinics, undertaking home visits where necessary, and managing a varied caseload.
- Demonstrating advanced clinical decision-making, assessment, and treatment skills.
- Acting as an independent prescriber, ensuring the safe and effective use of medications.
- Contributing to the development and implementation of new models of care and clinical pathways.
- Playing a key role in clinical governance, audit, and quality improvement activities within the practice.
- Mentoring and supporting the professional development of other members of the clinical team.
Key Skills:
- A Master's degree (MSc) in Advanced Clinical Practice or equivalent.
- Current registration with the NMC or HCPC.
- An independent prescribing qualification (V300).
- Significant post-registration experience in a primary care, urgent care, or acute setting.
- A proven ability to work autonomously and make complex clinical decisions.
- Excellent communication, interpersonal, and organisational skills.
- A commitment to continuous professional development and evidence-based practice.
- Experience with the EMIS clinical system is desirable but not essential.
