Summary: This role seeks an experienced Learning Disability Nurse to conduct joint reviews of care packages for individuals with Learning Disabilities and Autism in partnership with the local authority in Lancashire. Key Responsibilities:

  • Undertake detailed reviews of care packages alongside the local authority representative.
  • Assess and evaluate current care provisions to ensure they meet the needs of service users.
  • Utilize experience with the Continuing Healthcare (CHC) framework, including completing CHC checklists where appropriate.
  • Plan and recommend changes to care packages where necessary, ensuring the best possible outcomes.
  • Collaborate closely with multi-disciplinary teams to provide holistic, person-centred care.
  • Apply local resource allocation scheme principles &/ or funding calculators as directed.

Key Skills:

  • Qualified Learning Disability Nurse or Registered Nurse with extensive Learning Disability & Autism experience with relevant NMC registration.
  • Experience in conducting reviews of care packages.
  • Ability to identify and implement necessary changes to care plans.
  • Strong understanding of the CHC framework and experience completing CHC checklists.
  • Strong communication and collaborative skills.
  • 1+ years of work experience with NHS Commissioning.

Are you an experienced Learning Disability Nurse looking for flexible and rewarding work? We have an exciting opportunity to join our team and work in partnership with the local authority in Lancashire, conducting joint reviews of care packages for individuals with Learning Disabilities and Autism.

Key Responsibilities:

  • Undertake detailed reviews of care packages alongside the local authority representative.
  • Assess and evaluate current care provisions to ensure they meet the needs of service users.
  • Utilize experience with the Continuing Healthcare (CHC) framework , including completing CHC checklists where appropriate.
  • Plan and recommend changes to care packages where necessary, ensuring the best possible outcomes.
  • Collaborate closely with multi-disciplinary teams to provide holistic, person-centred care.
  • Apply local resource allocation scheme principles &/ or funding calculators as directed.

Role Options: Full-time, part-time positions or ad hoc available. Face-to-face reviews with remote write up. Travel and accommodation expenses included, where appropriate. Payment per case.
