Job Title: Social Worker - Hospital Based
Location: Worthing Hospital, West Sussex
Pay Rate: 35 - 38 per hour + 150 per week expenses (if based outside West Sussex)
Working Arrangements: Full-Time, 5 Days Per Week, On-Site

Job Overview:

We are seeking a dedicated Social Worker to join the Hospital Social Work Team at Worthing Hospital. This role offers the opportunity to make a positive impact in the lives of West Sussex residents by providing vital support to individuals in the hospital and those recently discharged. You will work closely with a multidisciplinary team, ensuring that individuals receive the care and assistance they need to maximise their independence.

Key Responsibilities:

Provide a statutory social work service, responding within agreed timescales.
Conduct strengths-based assessments for individuals in the hospital or recently discharged, and create tailored care plans.
Work collaboratively with hospital staff, family members, and external agencies to deliver person-centred care.
Support individuals in regaining skills and independence following their hospital stay.
Participate in family meetings, undertake ward visits, and contribute to multi-disciplinary discussions.

Compensation Details:

Base Pay: 35 per hour.
Expenses: 150 per week if you live outside West Sussex (subject to confirmation).
Please note, while West Sussex offer up to 38 per hour for those with over 2 years of qualified experience, pay is assessed based on skills and competency at interview, and may vary.

Essential Skills and Experience:

Demonstrated ability to make strengths-based assessments, implement care plans, and work in partnership with carers and external agencies.
Experience applying a risk enablement approach and using evidence-based practice.
Ability to analyse and interpret data to prioritise needs and formulate intervention plans.
Previous involvement in safeguarding enquiries.
Ability to mentor and coach less experienced practitioners and students.
Strong communication and interpersonal skills for effective stakeholder engagement.
Ability to manage responsibilities, prioritise workload, and seek support when needed.
Sound IT skills for maintaining accurate records, ensuring data protection and confidentiality.
