Summary: The Head of Community Palliative Care Services will lead and manage the community palliative care team within a large healthcare organization, ensuring the delivery of high-quality patient care. This role involves strategic planning, performance management, and fostering relationships with various health service providers. The successful candidate will be responsible for regulatory compliance and continuous improvement of service delivery. A strong background in community nursing or palliative care is essential for this leadership position.

Key Responsibilities:

  • Overall accountability for the Integrated Community Palliative Care Team, providing operational and strategic business planning and leadership of the services, dealing with all practice issues, making sure deadlines are met;
  • Managing key performance indicators (KPIs) and service measures ensuring high-quality patient care is delivered;
  • Motivating and managing the community palliative care team including nurses, healthcare assistants, Clinic and outreach staff including Allied Health Professionals (AHPs) and community engagement leading on the management of the shared community caseload;
  • Building and improving relationships, communication and teamwork with health service providers across the hospice and externally, including GPs and hospitals, raising awareness of the organisation and promoting its function;
  • Ensuring regulatory compliance with Care Quality Commission (CQC) standards and implementing and monitoring action plans for internal CQC audits and investigations;
  • Regularly reviewing and communicating risks with staff and identifying areas for improvement; contributing to investigations related to Serious Untoward Incidents or Duty of Candour incidents.

Key Skills:

  • Current registration to the NMC as a Nurse; advantageous if holding a post grad in community nursing or palliative care;
  • Demonstrated track record of managing multidisciplinary teams within a community setting;
  • Understanding of life limiting illnesses and pathways related to adult-based care.
