Summary: The Early Help Family Support Practitioner role involves providing tailored support to families with complex needs in East London. The practitioner will deliver interventions, coordinate multi-agency services, and assist parents to enhance outcomes for their children. This position requires collaboration with various professionals and adherence to child protection policies. The role is focused on improving child development through targeted parenting support and effective service coordination.

Key Responsibilities:

  • Assess family needs, create support plans, and provide targeted parenting support through home visits and group work.
  • Act as Lead Professional, collaborate with health, education, and social care services, and lead Team Around the Child meetings.
  • Ensure child protection policies are followed, maintain accurate case records, and adhere to GDPR regulations.
  • Deliver parenting programmes, attend multi-agency meetings, and work flexibly, including occasional evenings or weekends.

Key Skills:

  • Experience working with children and families, delivering parenting support, and providing targeted interventions.
