Summary: The Early Help Family Support Practitioner at Hackney Council is responsible for delivering interventions to families with complex needs as part of a multi-agency system. The role involves conducting holistic family assessments, promoting child and parent attachment, and coordinating support services to enhance family resilience and reduce isolation. Practitioners will work closely with families to create individualized plans and facilitate parenting groups to improve parenting skills and child outcomes.

Key Responsibilities:

  • Work as an Early Help Family Practitioner in accordance with Hackney Family Support Practice Guidance and Multi-Agency Team protocol.
  • Promote a safeguarding environment for children and ensure adherence to safeguarding procedures.
  • Provide structured support to children and families using assessment tools and create individual plans with agreed outcomes.
  • Act as lead professional for families, coordinating information sharing at MAT Meetings and team around the child meetings.
  • Support parents/carers through targeted work in the home, group work, and addressing barriers impacting parenting.
  • Coordinate and deliver parenting groups to enhance understanding of children's needs and strengthen parenting skills.

Key Skills:

  • Experience in family support or social work.
