Job Summary Hereford City District Nursing Team Have you always wondered what community nursing entails? Would you like a new challenge or have you had experience on the community and wish to pursue it as a career, then this is an opportunity for you. Due to the promotion of the current post holder we have a full time Band 5 community staff nurse vacancy within City District Nursing Team. As a Community Nurse you will become highly skilled in many different areas using many extended clinical skills and where no two days are the same. The team supports patients registered with the 2 Primary Care Networks which are co-located in the City of Herefordshire. It is a large team with a caseload of around 850 patients, this allows a higher level of support for staff new to the community, allowing supervision for competency achievement. Our base is at Belmont and we share a large office with neighbourhood team therapists which enables excellent levels of communication regarding shared patient care and concerns.

Main duties of the job Most of our work is with housebound patients but, we also run a District Nurse Clinic for non-housebound patients who need the very specific skills of a District Nurse to support their treatment. The focus of District Nursing is to support patients towards self-management, enabling them to continue to live as full and productive a life as possible, through education, equipment and or lifestyle changes. We also have the privilege of supporting people and their families at the end of their lives, working in close liaison with the Hospice Teams and Specialist Palliative Care Nurses. As community teams we enjoy working with many other health and social care professionals, Care Homes and Care Agencies. All staff are expected to record their work electronically on EMIS, our electronic patient record system which links into the GP services to allow a seamless service provision. Training and on-going support will be given to support staff with EMIS and other electronic systems they will need to use.
