Key Responsibilities:
- Provide urgent assessments and risk management for individuals experiencing a mental health crisis.
- Deliver evidence-based interventions and care planning to stabilise service users and prevent hospital admission.
- Work closely with inpatient services, community mental health teams, primary care, social services, and other stakeholders.
- Participate in daily clinical reviews and multidisciplinary handovers.
- Maintain accurate documentation and complete all clinical records in accordance with professional standards.
- Promote a recovery-focused and person-centred approach.
- Support discharge planning and step-down care where appropriate.
Key Skills:
- NMC registration as a Mental Health Nurse (RMN)
- Proven experience in a crisis or acute mental health setting (Band 6 level)
- Strong assessment, risk management, and decision-making skills
- Full UK driving licence and access to a vehicle for community visits (unless exempt)
- Able to work independently and as part of a multidisciplinary team
- Enhanced DBS (preferably on the Update Service)
- Experience working in Crisis Resolution or Home Treatment Teams (desirable)
- Knowledge of Isle of Wight community services and pathways (desirable)

Role Purpose: We are currently recruiting an experienced Band 6 Community Psychiatric Nurse (CPN) to join the Crisis Resolution Home Treatment Team (CRHTT) based at St Mary’s Hospital, Isle of Wight. This is a vital role within the Mental Health and Learning Disabilities division, providing urgent care and support to individuals in acute mental health crisis. The CRHTT practitioner will be a key member of a multi-disciplinary team, delivering short-term, intensive interventions to service users in the community as an alternative to hospital admission.
Key Responsibilities:
- Provide urgent assessments and risk management for individuals experiencing a mental health crisis.
- Deliver evidence-based interventions and care planning to stabilise service users and prevent hospital admission.
- Work closely with inpatient services, community mental health teams, primary care, social services, and other stakeholders.
- Participate in daily clinical reviews and multidisciplinary handovers.

Shift Pattern: This role operates on a 24/7 rota, including:
- Days
- Nights
- Weekends
- Bank Holidays

Essential Requirements:
