Summary: The Community Mental Health Senior Practitioner role in Tower Hamlets involves managing a complex caseload of adults with mental health needs, conducting assessments, and leading safeguarding investigations. Key Responsibilities:
- Manage a complex caseload of adults with mental health needs
- Complete Care Act assessments, risk assessments, and support planning
- Lead on safeguarding investigations and Mental Capacity Assessments
- Work closely with NHS, housing, voluntary sector, and other professionals
- Support service users under the Care Programme Approach (CPA)
- Provide mentoring and guidance to junior staff and social workers
- Prepare court reports, attend tribunals, and contribute to legal proceedings
- Ensure adherence to Mental Health Act, Care Act, and relevant legislation
Key Skills:
- Qualified Social Worker (SWE registered)
- Minimum of 2+ years post-qualifying experience in mental health services
- Knowledge of Care Act 2014, Mental Capacity Act, and Mental Health Act
- Experience working with individuals under CPA & S117 aftercare
- Ability to work autonomously in a fast-paced environment
- Experience in safeguarding adults and multi-agency working
