Summary: The Community Mental Health Practitioner in the Access Pathway plays a vital role in assessing and addressing the mental health and social care needs of referred individuals. This position requires strong risk assessment skills and effective communication to facilitate service provision and ensure high standards of care. The role involves working closely with crisis teams and requires participation in training and development initiatives. The practitioner will work part-time, alternating between evening and weekend shifts.
Key Responsibilities:
- Process and assess mental health and social care needs of referred individuals.
