Key Responsibilities:

• Manage a caseload of service users with complex health and social care needs.
• Conduct assessments, planning, implementation, and evaluation of evidence-based care.
• Coordinate care and assessments under the Care Programme Approach.
• Ensure compliance with Safeguarding Vulnerable Adults and Children Legislation.
• Adhere to the Mental Health Act 1983 requirements.
• Promote carer and service user involvement within the service.
• Establish therapeutic relationships and implement evidence-based therapeutic interventions.
• Deliver carers’ assessments, education, and support in line with NSF requirements.
• Provide support and caseload coverage for colleagues/team members.
