- Registered Learning Disability Nurse (RNLD) or Registered Nurse with relevant experience
- Valid NMC registration
- Experience within community learning disability services is highly desirable
- Strong assessment, care planning and risk management skills
- Ability to work autonomously in a community environment

Service Care Solutions are recruiting on behalf of one of our NHS clients for an experienced Community Learning Disability Nurse to join their team based in Preston. This role offers a stable Monday–Friday schedule, excellent support from the service, and the chance to make a significant impact within community settings.
Role Overview
The successful candidate will work within the Community Learning Disability Team, supporting individuals with a range of learning disabilities and associated health needs.
- Managing a caseload of individuals with learning disabilities within the community
- Completing comprehensive assessments and developing personalised care plans
- Providing specialist nursing input around physical health, mental health, and behavioural needs
- Working collaboratively with families, carers, multi disciplinary professionals and external agencies
- Ensuring high quality documentation and adherence to NHS and safeguarding procedures
- Delivering evidence based interventions to promote independence and wellbeing
This position requires strong clinical judgement, excellent communication skills, and a compassionate, person?centred approach.

Monday to Friday
9:00am – 5:00pm
Full time, ongoing NHS contract
