Service Care Solutions are recruiting on behalf of one of our NHS clients for an experienced Community Learning Disability Nurse to join their team based in Preston. This role offers a stable Monday–Friday schedule, excellent support from the service, and the chance to make a significant impact within community settings.

Role Overview

The successful candidate will work within the Community Learning Disability Team, supporting individuals with a range of learning disabilities and associated health needs.

  • Managing a caseload of individuals with learning disabilities within the community
  • Completing comprehensive assessments and developing personalised care plans
  • Providing specialist nursing input around physical health, mental health, and behavioural needs
  • Working collaboratively with families, carers, multi disciplinary professionals and external agencies
  • Ensuring high quality documentation and adherence to NHS and safeguarding procedures
  • Delivering evidence based interventions to promote independence and wellbeing

This position requires strong clinical judgement, excellent communication skills, and a compassionate, person?centred approach.

Working Hours

Monday to Friday

9:00am – 5:00pm

Full time, ongoing NHS contract

Pay Options

£26 per hour – Umbrella
