Summary: The Community Connector Mental Health role involves supporting GP surgeries in the East Waverley area by facilitating access to mental health services and community resources. The position is integral to the development of integrated primary care mental health services, requiring collaboration with various healthcare professionals. The role emphasizes early intervention and support for patients with mental health needs. This is a fixed-term contract for maternity cover, offering a hybrid working arrangement.
Key Responsibilities:
- Support GP surgeries in managing patients with mental health presentations.
- Coordinate and facilitate access to community services and resources.
- Work collaboratively with multidisciplinary teams including GPs, nurses, and mental health practitioners.
- Promote early intervention, triage, assessment, and care planning for patients.
- Engage with patients to help them focus on achievable goals.
Key Skills:
- Community experience in drug, alcohol, and mental health work.
- A Diploma or degree in a relevant subject (Counselling, Psychology, Mental Health, Social Work, Health and Social Care).
- Experience in signposting to community support services.
- Strong written and verbal communication skills.
- Proficiency in Microsoft Word and Excel.
- Ability to travel within the area, with own transport preferred.
