Summary: The role of Band 6 Community Psychiatric Nurse (CPN) in the Addictions Team involves providing specialized mental health and substance misuse support in Manchester. The position requires conducting assessments, developing care plans, and collaborating with a multidisciplinary team to ensure holistic patient care. The contract is ongoing with flexible working hours from Monday to Friday. Immediate start is available for qualified candidates.
Key Responsibilities:
- Conducting comprehensive assessments for individuals experiencing substance misuse and co-occurring mental health conditions.
- Developing and implementing tailored care plans in collaboration with a multidisciplinary team.
- Providing harm reduction advice, relapse prevention strategies, and crisis intervention.
- Supporting service users with medication management, detox support, and mental health interventions.
- Liaising with families, social workers, addiction services, and other professionals to ensure holistic patient care.
- Maintaining accurate and up-to-date patient records in line with NHS and addiction service guidelines.
Key Skills:
- A valid NMC registration as a Mental Health Nurse (RMN).
- Previous experience in substance misuse and addiction services.
- Strong assessment, risk management, and crisis intervention skills.
- The ability to work autonomously and within a multidisciplinary team.
