Summary: The role of Band 7 Therapist within the Home First Team at St Peter's Hospital involves leading service improvement and innovation in a multidisciplinary environment. The successful candidate will manage a specialist caseload, facilitate timely discharges, and contribute to the development of therapy services while supervising junior staff. This position emphasizes collaboration with various healthcare services to enhance patient care for older adults. Key Responsibilities:

  • Act as a specialist Therapist ensuring high standards of practice across all areas covered.
  • Manage the daily operations of the Home First Team and enhanced MDT to ensure patient flow.
  • Conduct highly specialist assessments and interventions for complex patients.
  • Make clinical judgments regarding risk-weighted discharge planning.
  • Facilitate timely discharges in collaboration with other MDT members.
  • Provide education and teaching to therapists and the wider MDT.
  • Supervise junior staff and students from Physiotherapy and Occupational Therapy disciplines.
  • Participate in the 'Front of House' weekend rota.
  • Contribute to improvements in Therapist roles and service delivery in ED.

Key Skills:

  • HCPC registration and a Diploma/Degree in Physiotherapy or Occupational Therapy.
  • Experience working within the NHS, particularly in Urgent and Emergency Care and Elderly Care.
  • Advanced clinical skills and reasoning.
  • Excellent communication skills, both verbal and non-verbal.
  • Ability to work within a multidisciplinary team.
  • Knowledge of NHS systems, discharge pathways, and clinical governance.
  • Experience in teaching and assessing competencies of staff.
  • Evidence of ongoing professional development.
